The registrant, on his or her behalf, and on the behalf of the participant(s) identified in this registration, acknowledges, understands, and agrees to the following:

1. I represent that I am the parent or legal guardian of the participant(s) named in this registration, or I have obtained permission from the parent/legal guardian of the registrant named in this registration to execute this agreement on their behalf.

 

2. I agree that the participant(s) will follow all kitchen safety guidelines.

 

3. I acknowledge and understand that there are known and unknown risks associated with food preparation and the use of kitchen tools and equipment including but not limited to minor burns, scrapes, and cuts.

 

4. I attest that the participant(s) named are free of any known allergies or serious food aversions, unless otherwise stated in the registration.

 

5. I understand that videos and photos are taken during the course of events for the purpose of marketing at The Cookery and that the participant(s) image may be used, unless otherwise stated in the registration.

 

6. I acknowledge that closed circuit video is being recorded throughout the public areas of McDowell Hall.

7. I acknowledge that there may be short intervals of 10 minutes or less where the participant(s) may be supervised by a vetted volunteer.

8. I understand that refunds may be issued at the sole discretion of the instructor or Executive Director depending on the circumstances.  In some instances, the option to reschedule or credits for other offerings may be offered in lieu of refunds.
